Terms and Conditions for Privilege Card
1. The Privilege Card is valid for 1 year from date of official receipt.
2. The Privilege Card is non-transferable and must be used only by the Cardholder.
3. The Privilege Card must be presented at all times when availing of the services.
4. Unauthorized use shall invalidate the Privilege Card.
5. The benefits are available in Metro Manila Only.

Privilege Card Cardholders can enjoy the following benefits during confinements:
√ Access to the Insular Health Care’s Privilege Card network of providers in Metro Manila
√ Privilege Card rates for InPatient Professional Fees (PF)
√ Assistance from Insular Health Care’s team via 24/7 support


AVAILMENT PROCEDURES FOR INPATIENT ONLY


• With attending specialist:
1. Secure admitting orders from Insular Health Care accredited specialist. You may visit the following links in our website:
a. Accredited Doctors:

b. Accredited Hospitals:
www.insularhealthcare.com.ph/search-hospitals-and-clinics-pc/
2. Proceed to Admission Section
3. Present Privilege Card and admitting orders
4. Notify Insular Health Care Customer Care Representative (CCR) through the hotline numbers
5. CCR visits cardholder within 2 days after admission or notice to Insular Health Care of admission and computes for Insular Health Care Privilege Card rate
6. Pay the specialist based on Insular Health Care Privilege Card rate and the Coordinator rate when visited by the Insular Health Care Coordinator


• Without attending specialist:
1. Proceed to Insular Health Care Coordinator of the hospital and present Privilege Card
2. Secure admitting orders
3. Proceed to Admission Section and present Privilege Card and admitting orders
4. Notify Insular Health Care Customer Care Representative (CCR) through the hotline numbers
5. CCR visits cardholder within 2 days after admission or notice to Insular Health Care of admission and computes for Insular Health Care Privilege Card Rate
6. Pay the specialist based on Insular Health Care Privilege Card rate and the Coordinator rate when visited by the Insular Health Care Coordinator
