Below are the mechanics:

1. The Promo period is from January 5, 2015 – March 31, 2015.

2. In order to qualify, applicants should apply online through our website and must fill out the online form. Applicants will receive P100 worth of SODEXO Premium Pass Gift Certificate for every Php 2,000 paid excluding the one-time enrollment fee(s) and optional dental plan annual fees.

3. The said promo is applicable to all approved new online applications and paid online or through other available payment facilities stipulated below. Renewal accounts are not included. To know the exact membership fee, simply apply online at

4. The said promo applies to the annual or first modal fees (semi-annual or quarterly) only and does NOT apply to succeeding modal and Other Fees.

5. For the purchases under this promo, the applicant may use the following payment facilities:
a. Pay in cash or check or credit card at Insular Health Care’s Head Office or any of its satellite offices. All check payments are treated as deposit until such time the check clears.
b. Pay online using any valid Credit Card or by filling out the corresponding Credit Card Authorization Form.
c. Pay over-the-counter (OTC) in any of our accredited settlement banks.

Those who opt to pay via payment facilities a, b, and c, please indicate the application number as reference number. The Application number is generated for every successful online application.

6. After initial payment, the approval of the healthcare application will be processed in the next 7 working days. Once approved, applicants will receive their SODEXO Premium Pass Gift Certificate/s through registered courier within 10 working days after approval of application.

7. Upon turnover of SODEXO Gift Certificate/s to regiestered Courier, an email will be sent to the applicant containing their tracking number. All SODEXO Premium Pass Gift Certificate/s will be shipped to the registered mailing address provided in the application form.

8. All SODEXO Premium Pass Gift Certificate/s are non-convertible to cash, however, they are transferable.

9. When redeeming the Sodexo Premium Passes, Sodexo Terms and Conditions shall apply. Insular Health Care, Inc., will not be liable for any claim relating to the purchase of goods and/or services from Sodexo or its Partner Merchants.

10. If the qualified applicant for the promo does not receive any SODEXO Premium Pass Gift Certificate/s, they are given up to 60 calendar days from date of approval of their applciation to call the Insular Health Care Head Office at Tel. No. 02-8130131 local 8206. Otherwise, this will be forfeited in favor of the company with prior approval of DTI-FTEB SPD.

11. Management reserves the right to decide on cases such as refund, discontinuity and disapproval, where the above-mentioned rules and mechanics do not apply.

12. In the event of an approved refund and the SODEXO Premium Pass Gift Certificate/s has already been used, the applicant will be refunded based on the refund policy of the company LESS the amount of the SODEXO Premium Pass Gift Certificate/s under the promo.

13. ALL Employees of Insular Health Care, Inc. including relatives up to 2nd degree of Consanguinity or affinity are not qualified in the promo.
